Consultation Planning
Going to your Doctor’s office and learning that you have been diagnosed with a serious illness can be extremely difficult and nerve wrecking. Sometimes the shock and disbelief can cloud your mind, and you might not be able to understand or remember the other information your Doctor gives you. It’s times like these when you wish you had someone by your side. Someone who understands your wants and needs, someone who knows what questions to ask, someone who has the knowledge and experience to ensure you receive the best possible care. We can be that person for you.
As your Healthcare Consultant, we can help you plan and prepare for your healthcare needs. Here are some of things we can help you with:
- Preparing questions in advance for an appointment with the healthcare provider
- Gathering important medical history and other relevant information to share with the healthcare provider
- Maintaining a comprehensive file on the client’s medical history, profile and medications
- Drawing up a personal and goal-oriented plan based upon client’s needs and beliefs
- Prioritizing and recommending next steps

WHAT TO EXPECT:
Making the most of your limited time with the healthcare provider – Consultation Planning is a process that provides an opportunity for the patient to organise and prioritise their questions in a calm, comfortable environment before meeting with a healthcare provider.
Using a systematic approach, the consultation planner assists the client in clarifying what information they need to obtain from the physician to feel satisfied. The planner does not answer medical questions or give advice about health or treatment options during the interview but may encourage the client to list their questions on the plan for the physician.
After the interview, a typed flow chart is created, which includes not only the client's unique questions but information about their personal background and communication style as it pertains to their current health situation. The finished plan reflects the concerns and questions the client had at that particular point in time.
The client receives a copy of the plan within 3 business days or at least 24 hours before their appointment with their healthcare provider, whichever is earlier. The client may use this plan to take notes during the visit. If needed, the client may schedule another consultation to discuss further issues.
in4MED offers comprehensive one-to-one personalised services. Appointments are available 9am-5pm, Monday-Friday excepting public holidays. In person consultations are available within the Kitchener-Waterloo region. Consultations take place in the comfort of the client's own home or chosen location. If you reside outside the Kitchener-Waterloo region, we are happy to consult with you over Skype.
