Surveys: When you use the Platform, you may be asked to complete a survey evaluating your experience with your healthcare advisor and the Platform. If you choose to complete such an optional survey, you may be asked to provide your age. We may provide your survey results to your healthcare provider to help them improve their services, and to interested third parties. Any survey results or related information sent to your healthcare provider or third party is provided in aggregate form.
Contact Us: When you contact us with a comment, question or complaint, you may be asked for information that identifies you such as your name, email address along with additional information we may need to help us promptly answer your question or respond to your request, comment or complaint. We may retain this information to assist you in the future and to improve our customer service and service offerings. We also use personal information to establish and manage our relationship with you and provide quality service.
